Degree

Name:* Enter your name up to 50 characters. In the case of a multiword, state the whole (for example, Mary Magdalene). Please do not enter any additional information.

Surname:* Enter your last name in the range of up to 100 characters. In the case of a multiword surname, state the full surname. Please do not enter any additional information.

E-mail:* Enter your e-mail address for electronic communication in the correct format with a maximum of 254 characters.

Please enter a valid email address. This will make it easier for you to communicate with the financial administration. An e-mail with an activation link will be sent to the e-mail address provided. Without activation, it will not be possible to complete the registration process.

Telephone number: Enter your phone number in the international format e.g. +421 yyy xxx xxx (where yyy is the area code).

Country:* Select a country. By entering at least 2 characters, the corresponding items from the dial will be offered.

Town/City:* Select a municipality or enter a municipality of up to 100 characters. By entering at least 2 characters, the corresponding items from the code list of Slovak municipalities will be displayed.

ZIP Code:* Enter the ZIP code without a space in the range of max. 5 characters (the specified range applies to SR).

Street:* Enter a Street of up to 100 characters. If the village does not have street names, enter the village name.

House number:* Enter the House Number in the range of max. 30 characters.

Enter your birth number in the form xxxxxx/yyy resp. xxxxxx/yyyy.

Date of birth:* Enter your date of birth in the form dd.mm.yyyy.

Password:* Choose your password. Length of min. 8 characters, min. 1 character from set [a-z], min. 1 character from set [A-Z], min. 1 digit from set [0-9], do not use accents, characters < > or any spaces.

Confirm password:* Re-enter your chosen password.

Security element:* Enter the characters from the picture of security element.

Mismatch of entered and displayed characters of the security element.
